Case Management

Case Managers are an integral part of the HSS health care team. They specialize in providing our patients with:

Assistance with discharge planning and coordination of medical needs and, if indicated, referrals for physical therapy, rehabilitation, or home care programs after leaving the hospital.
Emotional support and counseling for patients and family members.
Consultation regarding concerns about coping with illness and/or disability.
Exploration of resources to assist with personal, financial, and social needs which may have been affected as a result of your need for surgery.

Our Case Managers can help you before, during, and after your hospital stay. We have a team of certified social workers and registered nurses who assist patients from pre-admission to post-discharge.
